1.   Preparation for Your Appointment

 

For new patients, to better assist you upon your visit, please print and complete the new patient questionnaire, 

demographics, HIPAA, and financial policy forms below and present them to our staff when arriving for your appointment.

2.  What to Bring

 

  • Health insurance information including authorizations or referrals

  • Identification and insurance cards

  • List of prescription medications and dosages

  • List of medications that you're allergic to

  • Recent test results related to your condition (including diagnostic imaging studies)

  • List of questions you may have

Medical Records

If you would like your medical records released, please download and fax the following completed and signed form to 650-558-1806. Please note that records request may take up to two full business weeks for completion.
