To ensure your initial visit is efficient and focused entirely on your care, please complete the necessary paperwork before arriving.

  • Health insurance information including authorizations or referrals

  • Identification and insurance cards

  • List of prescription medications and dosages

  • List of medications that you're allergic to

  • Recent test results related to your condition (including diagnostic imaging studies)

  • List of questions you may have

Medical Records

If you would like your medical records released, please download and fax the following completed and signed form to 650-558-1806. Please note that records request may take up to two full business weeks for completion.
